Our History

Founded to Address the Needs of Safety Net Hospitals

We began as an internal department within Memorial Healthcare System, one of the largest public health systems in the country, serving as the Administrative Services Organization (ASO) for their employees.
Approved as a Provider Service Network (PSN) and launched PSN services for three (3) safety-net hospitals: Broward Health, Memorial Healthcare System, and Jackson Health System.
Shortly after, we expanded and began serving as the ASO for the system’s uninsured population, South Broward Community Health Services.

2014

Established plan infrastructure to administer services for the uninsured population as a not-for profit LLC and created South Florida Community Care Network, LLC (SFCCN)
Completed accreditation from the Accreditation Association for Ambulatory Health Care (AAAHC)

2016

SFCCN created Community Care Plan (CCP) as a DBA

2019

Achieved NCQA accreditation
Expanded and began providing administrative services for Broward Health’s uninsured population

2020

Began providing subsidized Children’s Health Insurance Program (CHIP) and fully paid health insurance coverage through Florida Healthy Kids, a program serving children from age five through the end of age 18
Expanded and began providing administrative services for Health Care District of Palm Beach’s uninsured population – Medical and Behavioral Health programs

2021

Expanded our administrative services for Health Care District of Palm Beach’s uninsured population – Trauma program
Executed contract with Jackson Health System to provide administrative services for their uninsured population (2022 projected start)
